Summary
The Team Leader is responsible for coordinating and providing quality patient care based on the Mission and Guiding Principles of the Choices Healthcare. By supporting the philosophy of patient care, the Team Leader provides leadership to the interdisciplinary team (IDT). The Team Leader coordinates all Team activities, assures that the Team is acting in concert with all federal and state regulations, oversees the frequency of patient visits, facilitates staff development, provides feedback to the Director of Clinical Care regarding staff performance and validates the appropriateness of patient care management. Demonstrates flexibility in meeting the organization’s needs, the Team Leader assumes delegated responsibilities from the Director of Clinical Care for operational and personnel activities that relate to quality, cost, and outcomes of patient care.
Qualifications
Degree in Nursing with PA licensure
Minimum of three (3) years clinical experience in specific specialty area with a sound knowledge base
Must demonstrate strong leadership skills and clinical competency
